it may be hard to find many true bloopers in the episodes, since they tended to either stop and reshoot scenes (resulting in reels of outtakes) or to adlib them in, like they've often been known to do in the Pink Panther series... but as with almost any long-running series, bloopers did get through once in a while...
for instance, in Season 2's episode "The Decoy", in an early scene, the Chief identfies two seperate men consecutively as Agent 58...
what other bloopers have you noticed that somehow got through?

here's a tiny one... in "The Only Way to Die", Max is pacing back and forth in a room, and he opens a door... then, when he's pacing, he goes through the doorway, and you hear the sounds as if he's falling down a flight of stairs and crashing at the bottom before he comes walking back out of the doorway.. however, we can see that he did not actually make such a fall, because when he passes behind the door, the sound effects start, but you can see him walking past the crack between the door and its frame (where the hinges are), and then as soon as he passes that crack one direction, he passes it coming back the other direction, emerging from behind the door only a second or two after he supposedly hit the bottom of the staircase...

in Part 2 of "House of Max" (at least - i didn't look all that close in Part 1), it's obvious the dummies aren't wax because the (not "living") Hitler dummy blinks and flinches a couple times while the bad guys are talking...

Isn't it odd how you can watch something time and time again, and on the 20th viewing you can catch something like that that you've never noticed before!

in "Perils in a Pet Shop", when the guy opens the box where the tranq guns are and takes one out, you can see that the other one is on the right side of the box... after that shot, you don't get a look again until Max goes to get it out of the box to shoot the henchman, but when he reaches for it, you can see it has moved to the left side of the box...

How about this: In "The Little Black Book, Part 2", the Maestro goes to the lengths to put a silencer on the cannon, but then tells Max and his pal that they shouldn't bother screaming because the walls are completely soundproof. What was the point of the silencer? THEN, when the Maestro comes down the stairs, the cannon goes off with a loud boom. What happened to the silencer?

that is an interesting observation... not sure if it's a blooper so much as just a lapse in logic, but yeah... they obviously didn't think it all the way through, did they? ;D

I'm often amused when you can clearly see the face of Don Adam's double, or when it's clearly a dummy. not sure if they count as bloopers, but still.

here's one i noticed last night: in Diplomat's Daughter, The Craw-er, The Claw states that although he has kidnapped 8 blondes, he was only after one person, but he had to kidnap them all because "All Americans look alike to us." it's a funny enough line, but the one person he was trying to kidnap was Princess Ingrid of Scandanavia... he should have said, "All white people look alike to us."

not a blooper from the episodes, but a blooper from the DVD introductions by Barbara Feldon:
in the intro for S1 E8, "The Day Smart Turned CHicken" (Nov. 6, 1965), Barbara falsely informs us that this is the first episode in which we hear the line, "Sorry about that, Chief!"
in truth, i know the line was at least uttered in S1 E5 "Now You See Him, Now You Don't" (Oct. 16, 1965), when the Chief is trying to leave Max's apartment, and Max head-butts him with the door... i believe this is the first instance of it, but i'm not 100% sure...
i think someone just made a booboo when they were writing the script for Barbara... maybe what she should have said is that S1 E8 is the first episode in which we hear the famous "Missed it by that much" line, because i think that is true...

blooper from Season 1's " I'm Only Human" - when Max and 99 sneak into the pet spa, both are wearing black gloves on both hands... the gloves are still there when they are captured and chained to the wall, and you can still see the gloves on their hands when they start talking about how to escape... but after the villain opens the elevator shaft and Max says he has a plan, his left glove mysteriously goes missing, and he finishes the episode with only one gloved hand! what happened?!

just a little one, but in It Takes One to Know One, the Dr. says he's gonna check Hymie's heart, but after Hymie jumps on the examining bed/table, the Dr. forgets about it and Max says the Dr. wants to look in his ear...
also in that episode, Hymie as CONTROL's #2 man gives Max 10 demerits and tells him he's fired, but nobody takes it seriously at all...
just little things, but oddities to me as i was watching this time around...

ok, in Season 2's "The Mummy", when Mrs. Smith takes the knock out pill, she freezes, holding her drink in the air... when Max takes the drink out of her hand, her hand falls to her lap... but then when Max is knocked out and falls over, the sleeping Mrs. Smith has raised her empty hand back to the position as if she were holding her drink...
little things i'm surprised i catch sometimes...
